The Scheme supports GP practices to make sure that the carers are recognized and signposted to get the best information that can support them. This includes : information for the practice’s websites, including a poster, social media help, developed a carers information pack and provided information to go on the electronic screens in the GP practice waiting rooms. As well as setting up a referral pathway on EMIS to make it easier for both the carer and the GP. EMIS (Egton Medical Information System) is the software where all your medical history and information is kept. So if you make a doctors appointment and they become aware you are a carer, they can pull it up on EMIS and refer you through to the carers center where someone will get in contact with you to arrange a carers assessment and to show what activities we have on offer.
‘The Carer Passport Scheme within GP Practices aims to support Liverpool practices to meet the ambitions of the NHS as set out in the Long-Term Plan. It will do this by aiding practices in identifying and supporting carers and support them to understand and meet the requirements of the CQC Quality Markers’
Funding has allowed the recruitment of our carers GP Link Worker to run a pilot with GP practices in Liverpool to ascertain the willingness of practices to become involved in the identification and support of carers in their areas. From the feedback from this pilot, we have increased the list of support for the GP practices.
Chris has also visited flu clinics to raise awareness to speak to patients and carers. He has encouraged staff to identify themselves if they are in a caring role, attended health and wellbeing events and supported surgeries to set up their own carers support groups. Once they have been referred, they get a careers assessment so they can see which of the amazing activities we offer they will benefit from the best. From Yoga to calming crafts there is something for anyone, at all abilities. Out of the initial nine surgeries that have shown interest there have been 77 referrals. As of the 31st of December, we have 18 GP practices have shown interest and have requested to be involved. Due to this we have had to set up a traffic light system, the surgeries in green, have worked intensely with us to find out what works for the staff.
